Introducing v-safe

V-safe is a smartphone tool that uses text messaging and web surveys to perform personalized health checks after receiving a COVID-19 vaccine. Through v-safe, you can quickly tell the CDC if you have any side effects after receiving a COVID-19 vaccine.

This information helps the CDC monitor the safety of COVID-19 vaccines in near real time. Based on your responses to v-safe questions, someone from CDC can call to check on you or your child and get more information. V-safe will also remind you to receive extra doses of the COVID-19 vaccine if you or your child needs them.

Parents and guardians can enroll children aged 5 and over in v-safe and perform health checks on their behalf after being vaccinated against COVID-19. All family members eligible for vaccination can be enrolled in the v-safe. Parents and guardians must use their smartphone to complete a separate form v-safe registration for each child. All v-safe communications will be sent to the smartphone of the parent or guardian.

If you are subscribed to v-safe and report that you were pregnant at the time of or after vaccination, CDC COVID-19 * Pregnancy Registry staff may contact you to learn more. If you choose to enroll in the registry, you will be contacted several times throughout your pregnancy for additional health checks.

To schedule, postpone or cancel a COVID-19 vaccination appointment, contact the organization that made your appointment or a vaccinator in your area.

You will receive a COVID-19 vaccination record during your vaccination appointment. Learn more about COVID-19 vaccination cards, including what to do if you’ve lost or haven’t received your card.

If you have any symptoms or health issues that concern you at any time after getting the COVID-19 vaccination, please contact your health care provider. In addition, if you have not been able to report your post-vaccination experience in v-safe (due to a missed or expired health check), you can report adverse events after vaccination to the Vaccine Adverse Event Reporting Systemexternal icon.
